Brgy. Balubad Bulacan, Bulacan to be my true and lawful Attorney-in-Fact, for me and in my
name, place and stead, to do the following acts and deeds to wit:

1. To apply for the issuance, certification and authentication of the Certificate of Live
Birth of my child, _______________________________, my certificate of entry/arrival
and exit/departure records and any other documents with the relevant issuing, certifying
or authenticating office/s, including but not limited to the), the Philippine Bureau of
Immigration (B.I.), Department of Foreign Affairs (DFA), TAIPEI Economic and
Cultural Office in Manila (TECO) and TECO accredited translators, and other relevant
offices, as required by the National Labor Insurance of Taiwan, in order that I may
claim the MATERNITY BENEFITS;

2. To pay the required fees and charges; and to make, execute, sign, acknowledge and
deliver any and all documents that may be required by the relevant issuing office/s, BI,
DFA, TECO and other offices, for the processing, approval, and release of the above
mentioned document/s.

HEREBY GIVING AND GRANTING unto my said Attorney-in-Fact full of power and
authority to do and perform every act and things whatsoever required and necessary to be done
under the premises, for all intents and purposes as I might or could do if personally present, with
power of substitution and revocation, and hereby ratifying and confirming all the acts that our said
Attorney-in-Fact shall lawfully do or cause to be done by virtue hereof.
