I am Anne Margaret P. Camba, a BS-Nursing Level 1 student of Colegio de Sta. Lourdes of Leyte
Foundation, Inc. with an official address at Tabontabon, Leyte.

It has come to my attention that your good office is currently offering scholarships for financially
challenged yet deserving students at the tertiary level. Your office also provided some qualifications for
such a scholarship, one of which is that the student must be an exited 4Ps beneficiary. With this, I
decided to submit my application for the scholarship as I also wanted to lessen the financial burden on
my family as the three of us are currently studying in college. Being accepted into this scholarship would
be a big help, especially since I am in my second year next semester and my parents are struggling in
providing financial support for me as the study materials and books in my course are quite costly.

Attached with this letter are the requirements to be submitted as proof of my eligibility for the
scholarship you are currently offering.
