1 PURPOSE

The aim of this standard is to prescribe the requirements for the control and safe work on live
energized TMM for system integrity, operability, diagnostics, infra-red scanning and fault finding
such as, measuring voltage, currents, pressures and flows on hydraulic, pneumatic and electrical
systems.

2 SCOPE

The requirements of this standard are mandatory and apply to all Venetia Mine operations.

3 DEFINITIONS

TERM DEFINITION
The word “shall” is to be understood as mandatory and the word “should” as
“Shall”
non-mandatory, advisory or recommended.
Approved Approved by General Manager or designated representative.
A person who:
(a) (i) is qualified by virtue of his knowledge, training, skills and experience
to organise work and its performance
(ii) is familiar with the provisions of the Act and the regulations which apply
Competent Person to the work to be performed; and
(iii) has been trained to recognize any potential or actual danger to health
or safety in the performance or the work; or
(b) is in possession of the appropriate certificate of competency where such
certificate is required by these regulations.
Testing is a process whereby a competent person in charge of the work being
carried out shall, by means of a positive test, ensure that the TMM has been
Testing safely isolated before commencing work on the TMM, i.e. cleaning, servicing,
and replacing components and maintenance work. For testing as described
in this definition a Permit to Work is not required.
Fault finding is a process whereby a competent person uses instruments,
programmers, schematic diagrams, tools and manuals to find a problem on

energized TMM. The repair however must only be conducted after the TMMs
has been isolated and tested for zero potential.
For the purpose of this document “energized TMM” means any equipment
that has a function that can isolate the energy of the equipment but are in a

and monitoring purposes.

5 REVIEW RESPONSIBILITY
5.1 Section Engineer: Haul and Ancillary Equipment
It is the responsibility of the Section Engineer: Haul and Ancillary Equipment to review this standard
at least once every two years.

6 IMPLEMENTATION RESPONSIBILITY
The appointed Responsible Senior Manager Engineering (Reg. 2.13.1) is responsible to ensure that
this Standard is implemented.

7 STANDARD SPECIFICATION

7.1 People Requirements


7.1.1 Intent

Every person that could potentially be exposed to live circuits on energized TMM shall receive basic
job specific training in order to understand the practicalities and safe work practices of working on
energized TMM:
 Electricians & Millwrights – Auto Electrical Training.
 Mechanical – Basic Hydraulic training.
 OEM specific training on TMM and has their competency assessed on an annual basis by
means of a task observation by the Responsible Supervisor (2.9.2).

7.1.2 Responsible Supervisor (2.9.2) Role

The responsible supervisor is the person appointed in terms of applicable legislation or internal
regulations. The responsible supervisor shall:
 Ensure that all live work on energized TMM is carried out in terms of this standard.
 Authorize suitably competent persons in accordance with requirements.
 Ensure and record that all authorized persons remain competent (by means of observation
audits and re-training) to carry out their duties.
 Ensure that this procedure is adhered to by conducting verification exercises such as planned
task observations.

7.1.3 Responsible Section Engineer

 Ensure that the live work on energized TMM procedure and standard remains current and that
it is updated when necessary to provide for equipment and/or process modifications (follow
the Management of Change Procedure).

7.1.4 Person Performing Live Work

 Shall be competent
 Shall be responsible for the safe execution of duties as per the live work on energized TMM
standard.
 The live work of energized TMM task shall only be conducted by qualified individuals whose
competency has been regularly verified and permitted as such by management.
 No work on energised equipment shall be undertaken unless permitted to do so.

7.2 Live Work on Energized TMM Actions - General Safety


7.2.1 Accessing Live Energy Sources

 Where the energy source remains energized, only cubicles, electrical apparatus, panels,
pumps, pressure lines, minimess test points etc. being worked on may be open or exposed at
any one time for live work and must be made safe before leaving the area.
 Door interlocks, safety valves and check points may be override for the purpose of live work or
infra-red scanning with permission clearly stipulated on the permit.

7.2.2 Risk Assessment

 Before any work is performed, time shall be taken to assess the potential risk to ensure all
hazards have been identified and controlled.
 Determine the hazard Consequence and Likelihood by using the Anglo American Risk Matrix.
 Task identified as high risk shall be discussed with the Engineer and the method of performing
the live work shall be agreed to together with the Engineers approval of the Risk Assessment
or Procedure which will be followed. If a hazard is identified that cannot be easily and
adequately controlled, then the task shall be stopped and the process reviewed.
 The risk assessment and required controls shall also take into account the prospective level
of energies as example fault current, pressure, flow or intensity of energy.

7.2.3 Testing Equipment

 Care must be taken when test equipment is being used to ensure the safety of all personnel
in test area. After removal of the energy, equipment must be discharged to remove all stored
energy.
 Any section using test equipment shall ensure that the equipment is approved for use, is
suitably rated for the purpose and is within a calibration/testing period if required by the OEM.
 Particular attention shall be paid to the voltage, current, pressure and flow ratings of the
equipment.
 Test equipment shall be visually inspected before each use. This shall include an inspection
of all test leads, probes, pressure and flow meters and connection lines, to ensure they are in
a safe operating condition.
 Test equipment which is used to test for zero potential shall be tested on a known source
before and when possible after use.
 Testing probes, leads, crocodile clams and multi meter accessories and instruments should
be calibrated and declared safe as per OEM recommendation.

7.2.4 Area and TMM Safety

 Work areas shall be kept clean and free of slip and trip hazards.
 If left unattended with exposed de-energised energies, areas shall be barricaded off to prevent
entry by unauthorised people.
 Live exposed energies in unrestricted areas shall not be left unattended under any
circumstances. Panel doors, covers, pressure lines and test points shall be properly secured
at the completion of work and all excess material stored in the correct location.
 Signage shall appropriately be placed on the TMM indicating that live testing is in progress.
 Chock blocks shall be in place where applicable.
 The TMM shall be barricaded and/or demarcated as per Barricading of Unsafe Workplace and
Hazardous Area Recommended Engineering Standard DBG 0899.

7.2.5 Appropriate Clothing and Personal Protective Equipment (PPE)

EVERY PERSON who intends to work on or in close proximity to any energy system, whether the said
systems are alive or dead but capable of being made alive, shall be correctly dressed and shall wear
all relevant protective clothing (PPE) or equipment issued for the purpose.

7.3 Live Work Method


 Communicate with the supervisor, operations personnel and any other party as to the extent
and nature of live work to be performed.
 Perform a Risk Assessment.
 Complete the “Permit to Work on Energized TMM”
 Only appointed 2.13.1; 2.13.3.1; 2.13.3.2 Engineers or their authorised delegates are to
authorise this permit.
 Discuss the following with the competent co-workers:
o Roles and responsibilities with one person in charge of work
o Location of medical first aid kid
o Location of fire extinguishers
o Emergency exits
o Potential hazards
o Hand signals, spotting and communication methods.
 Identify the panels, cubical, transformers, electrical equipment, pressure/flow minimess test
points, pumps, accumulators etc. to which testing equipment, probes and meters are to be
attached. Always refer to schematic diagrams.
 Ensure all unnecessary personnel are clear of the test area. Barricade the area if necessary.
 Ensure all personnel wear the proper PPE before opening any energy source.

 Where a TMM operator is required such person shall be licenced. Where practical the operator
shall start the machine and during the live work process remain outside the cab of the TMM.
In cases where this is not practical such as for example taking VIMS readings, the operator
may remain in the TMM cab but shall take the necessary precautions not to set the TMM in
motion.
 When opening an electrical panel or cubical, make sure that the person opening the panel or
cubical has no tools or testing equipment in their hands that can fall into live bus bars in the
process.
 Close and latch any doors not required to be open and in the case of moving or surface mobile
equipment secure any door required to be left open to prevent them from swinging when
machine operates.
 Apply the clip, testing equipment, probes or minimess hose and ensure it is in contact/position
only with the required terminals or minimess test point.
 Testing may now be carried out
 Unclip or remove the probes or testing equipment and close and secure the panel, cubicle,
valves or any other item where testing equipment were applied to.
 Complete “Permit to Work on Energised TMM” form and file for later references.
