1. Overview
Contrast Enhanced MR Angiography (CE-MRA) allows you to acquire images of better
contrast by intravenously injecting a low invasive contrast agent (Gd-DTPA). CE-MRA
obtains contrast of blood vessels by T1 shortening effect of contrast agent, thus shortens scan
time. On the contrary, ordinary MRA using the TOF sequence acquires blood flow images
depending on velocity of blood flow and it is apt to miss signals at a bent or stricture, so it
cannot be a method to show true intravascular information. Also, it needs large amount of
images, that is, long scan time.

To visualize a target blood vessel of high contrast in CE-MRA, it needs to carry out scan at the
time when density of the contrast agent in the target blood vessel is the highest. In addition,
to improve visualization capability, it suppresses background signals by subtracting the image
before the contrast agent travels to the target blood vessel (Measurement 1 in Fig. 1.1) from
the image after contrast agent travels to the target blood vessel (Measurement 2 in Fig. 1.1).
Also, applying projection process (MIP) to the acquired subtraction image allows you to obtain
an image of higher contrast.

2. General Imaging Procedure
2.1 Overall Flow
Fig. 2.1.1 shows general procedure of CE-MRA imaging. After acquiring scanogram at two
planes, scan called the test injection is performed to obtain a time for the contrast agent to
travel to the target blood vessel.
By utilizing the time for the contrast agent to travel to the target blood vessel with the test
injection, the main scan can visualize the target blood vessel of high contrast.

2.2 Test Injection
To visualize the target blood vessel of high contrast in CE-MRA, timing of the main scan is
important. To obtain the optimum timing of this main scan, a scan called the test injection
is performed before the main scan. The test injection injects little amount of contrast
agent (1~2ml) to obtain Contrast Travel Time at the target blood vessel. A conceptual
diagram of this test injection is shown in Fig. 2.2.1.

Fig. 2.2.1 shows Tij to inject the contrast agent and Tt for the contrast agent to reach the
highest density in the target blood vessel. Contrast Travel Time at the target blood vessel is
the time for density of the contrast agent to reach its peak minus the time when the contrast
agent was injected (Tdelay in Fig. 2.2.1). The main scan allows the target blood vessel to be
visualized by measuring data Tdelay after injecting the contrast agent as a center of
measurement data space (k-space).

2.3 Main Scan
After obtaining Contrast Travel Time at the target blood vessel by the test injection,
perform the main scan. The main scan carries out scan of a reference image first and then
intravenous injection of contrast agent and scan of angiogram follow.
The main scan uses 3D GE RS-SARGE (RF Spoiled Steady state Acquisition with Rewound
Gradient Echo) sequence. The four types of ADA ( Asymmetric Data Allocation ) ,
PEAKS(PEak Artery enhancing K-space filling Sequence), RPEAKS(Reversed PEak Artery
enhancing K-space filling Sequence) can be selected as order of acquiring measurement
data in this sequence.

To obtain an image with high contrast, it is important to set measurement data when
density of contrast agent is the highest in the target blood vessel to the center of k-space.
Time from starting measurement to injecting the contrast agent is calculated considering
Contrast Travel Time at the target blood vessel. Therefore, time to inject the contrast agent
differs according to order of acquiring measurement data. The relation between
concentration of contrast agent and measurement in each order is shown below.

The conceptual diagram and order to acquire data in k-space of the main scan in full scan
are shown in Fig. 2.3.1(a) and Fig. 2.3.1(b) respectively. Optimum time to inject the contrast
agent is Tinjection after starting measurement of Fig. 2.3.1(a).

The conceptual diagram and order to acquire data in k-space of the main scan in ADA are
shown in Fig. 2.3.2(a) and Fig. 2.3.2(b) respectively. The area of first quarter is not
measured to shorten time to reach the center of k-space. Thus, acquiring data in k-space
can be finished within the time zone when concentration of the contrast agent has reached
the target blood vessel.
Optimum time to inject the contrast agent is Tinjection after starting measurement of Fig.
2.3.2(a).

The conceptual diagram and order to acquire data in k-space of the main scan in PEAKS
are shown in Fig. 2.3.3(a) and Fig. 2.3.3(b) respectively. PEAKS are "the 2D Ordering
methods of ky-kz plane which has the feature of Sequential", and improve 3D k-space order.
Order is set on the basis of the distance between the center of the ky-kz plane and each
measuring point. In the time zone when concentration of the contrast agent in the target
blood vessel have not reached the peak, data is acquired from the farther points to the
nearer(region ① in Fig.2.3.3(b)). In the time zone when concentration of the contrast agent
is about to reach the peak, data of vicinity of the center is acquired(region ② in
Fig.2.3.3(b)). And in the time zone when concentration of the contrast agent have passed
through the peak, data is acquired from the nearer points to the farther(region ③ in
Fig2.3.3(b)). Controlling 3D k-space order like this enable you to obtain images with
capability to suppress the signal of vein and less influence of artifacts caused by time gap.
PEAKS are effective for diagnosis of the region with fast velocity of blood flow, for example,
the cervical region, which demands sufficient arterial to venous separation and high special
solution.
Optimum time to inject the contrast agent is Tinjection after starting measurement of Fig.
2.3.3(a).

The conceptual diagram and order to acquire data in k-space of the main scan in RPEAKS
are shown in Fig. 2.3.4(a) and Fig. 2.3.4(b) respectively. RPEAKS is on the same basis of
principle as that of PEAKS. Order is set on the basis of the distance between the center of
the ky-kz plane and each measuring point. In the time zone when concentration of the
contrast agent in the target blood vessel have not reached the peak, data is acquired from
the farther points to the nearer(region ① in Fig.2.3.4(b)). In the time zone when
concentration of the contrast agent is about to reach the peak, data of vicinity of the center
is acquired(region ② in Fig.2.3.4(b)). And in the time zone when concentration of the
contrast agent have passed through the peak, data is acquired from the nearer points to the
farther(region ③ in Fig2.3.4(b)). The center of k-space is measured in the later half of the
measurement in RPEAKS compared with that of PEAKS. Thus, when performing Multi
Station Measurement (option), PEAKS or RPEAKS can be selected in each station, so that
acquiring data in k-space can be finished in a optimum way within the time zone when
concentration of the contrast agent has reached the target blood vessel in each station.
Optimum time to inject the contrast agent is Tinjection after starting measurement of Fig.
2.3.4(a).

3. Measurement Operation
3.1 Test Injection
3.1.1 Setting Basic Group
Carry out the test injection after acquiring a scanogram.
(1) Select the Scan Card.
(2) Select 2D from Acquisition Mode, GE from Sequence Name and RSSG from Scan
Option of the Scan Card.
The Basic group is displayed as shown below.
Refer to the standard protocol and instruction manual of main unit to set each
parameter to any desired value.

Click the Dynamic tab to set each parameter.

(1) Since the contrast agent is used, set Cont. Agent to Gd-DTPA.
(2) Since scan is performed in the Dynamic Scan, set Dynamic to Auto or Manual.
(3) Refer to the standard protocol and instruction manual of main unit to set parameters
concerning Dynamic Scan. (Even in the test injection, subtraction image can be
obtained. Parameters Sub.Image and Src.Image newly added in accordance with
subtraction image will be described in Section 3.2.3.)

3.1.3 Starting Stop Watch


(1) Select Stop Watch from the system tool. The following Stop Watch dialog starts.

(2) Check Sync or Async of Stop Watch parameter, and press the START button of Stop
Watch.
The measurement procedures of Sync and Async are different. These procedures are
described in the next item.

3.1.4 Starting Measurement
A. In case of Stop Watch being set at Sync
(1) Press the Start button of Scan Card to start scan.
(2) After completing prescan, display of the Start button changes to Cont. Press the
Cont. button to start measurement. As soon as measurement starts, the Stop Watch
is initiated. Determine any time displayed on the Stop Watch and inject the
contrast agent. When selecting Manual for Dynamic, press the Cont. button any
time after injecting the contrast agent.
(3) After confirming the contrast agent travels to the target blood vessel, press the Stop
button to terminate the measurement.
(4) The time for the contrast agent to arrive at the target blood vessel is the time that
the Stop Watch time when injecting the contrast agent determined in (2) is
subtracted from the Stop Watch time in the ID information column at the upper right
of screen, when the contrast agent is confirmed to travel to the target blood vessel
(See Fig. 3.1.4.1).
(5) Press the RESET button of Stop Watch.

B. In case of Stop watch being set at Async


(1) Press the Start button of Scan Card to start scan.
(2) After completing prescan, display of the Start button changes to Cont. Press the
Cont. button to start measurement. After starting the measurement, inject the
contrast agent any time. At the same time as the injection of contrast agent, press
START button of the Stop Watch. The Stop Watch is initiated. When selecting
Manual for Dynamic scan, press the Cont. button with appropriate timing after
injecting the contrast agent.
(3) After confirming the contrast agent travels to the target blood vessel, press the Stop
button to terminate the measurement.
(4) Confirm the Contrast Travel Time at the target blood vessel. The time of Stop
Watch displayed in the ID information column when confirmed the travel of contrast
agent at the target blood vessel is the duration of time from injection of the contrast
agent to travel to the target blood vessel. (See Fig. 3.1.4.2.)
(5) Press the RESET button of Stop Watch.

3.2 Main Scan
3.2.1 Setting Basic group
After the time for the contrast agent to travel to the target blood vessel is obtained by
the test injection, perform the main scan.
(1) Select the Scan Card.
(2) Select 3D from Acquisition Mode, GE from Sequence Name, RSSG from Scan Option
and SQ#2 from Sequence No. of the Scan Card.
The Basic group displays as shown below.
Refer to the standard protocol and instruction manual of main unit to set each
parameter of Basic group to any desired values.

Click the Particular tab and set parameters.

(1) E.Alloc.
Select echo allocation method.
In 3D GE RSSG for the main scan sequence of contrast enhanced MRA, four types of
Sequential, ADA (Asymmetric Data Allocation), PEAKS(PEak Artery enhancing
K-space filling Sequence) and RPEAKS(Reversed PEak Artery enhancing K-space
filling Sequence) can be selected. Only in this sequence, ADA varies its process to be
executed for the area of remaining 1/4 depending on combination with the Half Scan
method. If selecting Half Scan On3/4 in E.Alloc. ADA, an area of 1/4 not measured is
determined by estimation from measured area. If selecting Half Scan off, the area of
1/4 not measured is not estimated (See Fig. 3.2.2.2). When determining the area by
estimation, spatial resolution is maintained but S/N is deteriorated. When not
determining the area not measured, spatial resolution is slightly deteriorated but S/N
is improved.
PEAKS and RPEAKS set the area of not measured depending on combination with
APR(Asymmetric Projection Rate). The available percentage of APR is 0 to 25 %.
Besides, PEAKS and RPEAKS vary its process to be executed for the area of not
measured depending on combination with the Half Scan method. The process is the
same as that of ADA (See Fig. 3.2.2.3 and Fig. 3.2.2.4).

3.2.3 Setting dynamic group
Click the Dynamic tab to set each parameter.

(1) Contrast agent is used, so set Cont. Agent to Gd-DTPA.


(2) Dynamic Scan is used, so set parameter Dynamic to Auto or Manual.
(3) Refer to standard protocols and the instruction manual of main unit to set
parameters.
(4) Travel Time
Enter Travel Time of the contrast agent to the target blood vessel. Enter that
obtained from test injection.
(5) Injct SWT
Displays time from scan start to contrast agent injection. If entering Travel Time,
this is automatically calculated taking account of parameters such as E.Alloc.
Time of Injct SWT is calculated by the following equation according to combination of
E.Alloc and Half Scan.

(6) Scan SWT
Displays time from contrast agent injection to scan start. Displays only when
parameter Dynamic is Manual and Int. Pause is off. It takes a value other than 0 in
such a case as pressing the Cont. button to continue the scan after contrast agent
injection. If Scan SWT becomes that other than 0, Injct SWT will be 0.

(7) Int.Pause
This is a parameter to pause scan when the scan of 1st Number is completed in the
dynamic scan. In either case of Dynamic Auto or Manual, ON may be selected. If
selecting off, normal dynamic scan is effected.
If setting parameter Dynamic of the Scan card to Auto, scan pauses automatically
after the scan of 1st Number is completed. To continue the scan, press the Cont.
button. After pressing the Cont. button, scans after 2nd Number are effected (1st
Interval – scantime)[sec] later. Operation after this is the same as that of Dynamic
Scan Auto.
If setting parameter Dynamic of the Scan card to Manual, the same parameters as
those of Dynamic Auto; 1st Number, 1st Interval, 2nd Number, 2nd Interval, 3rd
Number and 3rd Interval are allowed for entry. If starting scan, as well as normal
Manual scan, the scan pauses after completing the scan of 1st Number. To continue
the scan, press the Cont. Button. For scans after 2nd Number, the scan continues
(Interval in question – scantime)[sec] after pressing the Cont. button.
Figs 3.2.3.3 and 3.2.3.4 shows the function when Int.Pause is turned on.

(8) Sub.Image
Select Subtraction Image creation. If selecting Fixed, subtraction images are
created with the 1st Dynamic scan as a mask image. If selecting Moved,
subtraction images are created with the mask image changed. If selecting Fx/Mv,

selecting any parameter other than off, the 1st Dynamic Scan creates a source image

source image is created. When Sub.Image is off, ON only can be selected for

3.2.4 Starting scan
If no Stop Watch has started, select the Stop Watch from the system tool.
A. To perform scan with Stop Watch set to Sync
<Dynamic: Auto, Int. Pause: off>
(1) Press the START button of Stop Watch. Display of Stop Watch turns to
“Waiting…”.
(2) Press the Start button of Scan card to start scan.
(3) After completing prescan, press the Cont. button. Main scan starts and Stop
Watch is activated.
(4) Inject the contrast agent when time of Stop Watch reaches the value of Injct SWT
in the Scan card.

<Dynamic: Auto, Int.Pause: ON>


(1) Press the START button of Stop Watch. Display of Stop Watch turns to
“Waiting…”.
(2) Press the Start button of Scan card to start scan.
(3) After completing prescan, press the Cont. button. Main scan starts. At this
time, no Stop Watch works.
(4) The scan pauses after completing the scan of 1st Number. Press the Cont.
button to continue the scan. At this time, the Stop Watch works.
(5) Inject the contrast agent when time display of the Stop Watch reaches the same
value as that of Injct SWT.

<Dynamic: Manual, Int.Pause: off>


The case the Stop Watch is allowed to be Sync is only when parameter Scan SWT of
the Scan card is 0. When Injct SWT is 0, perform scan with Stop Watch set to
Async.
(1) Press the Start button of Scan card to start scan.
(2) After completing prescan, press the Cont. Button. Main scan starts.
(3) After scanning a mask image is completed, press the START button of Stop
Watch.
(4) Pressing the Cont. button activates Stop Watch. Inject the contrast agent when
time display of the Stop Watch reaches the value of Injct SWT.

value of that of Scan SWT.

3.2.5 Starting Creating MIP images
This procedure applies to just after completing the main scan. If the MIP process is
to be applied to any image in the database, create the Data card and display image
data of database on the Control Face of Data card before processing.
(1) Click the Pick Images button to display Image Selection.
(2) Click the Sorting button of Image Selection. (For Image Selection, refer to Section 4
in the instruction manual of main unit (Operation)).
(3) Select attributes from Sort Attributes in the order of Type and Image Nr. And click
the Done button. If setting parameter Src.Image to ON and Sub.Image to Fx/Mv,
the order to display images is changed as shown below:

(4) Select any desired image and click the Include or Exclude button. If using the Data
card, press the Start button.
(5) If opening the 3D card, the MIP process is effected. (For the 3D card, refer to the
instruction manual of main unit.)

4. Cautions
(1) Since the subtraction process is carried out, secure the patient not to move all one can to
prevent an effect of body motion.
(2) Total volume of the contrast agent and salient used and injection speed should be as same
as possible both for the test injection and main scan. Making them the same allows you
to visualize the target blood vessel of high contrast. If varying total volume and injection
speed, Travel Time can change resulting in deteriorated contrast of the target blood vessel.
