APPERANCE RELEASE FORM

I, (name), hereby grant to Danielle White the right and permission to photograph me, record my
performances, poses, actions, and appearances and other reproductions of my physical likeness
in connection with the short film entitled “Who’s Next?” I acknowledge and understand these
materials about or of me may be shown to the class for educational purposes.

I understand that my image may be edited, copied, and exhibited. I further grant the right to
reproduce any recording whatsoever, including all instrumental, musical, or other sounds
produced by me, in connection with the production and/or postproduction of this short film.

I hereby certify that I am over the age of eighteen and take full responsibility for my involvement
in this project and the risks that it may entail (be they legal, physical, or mental). Additionally, I
understand that I will adhere to the schedule and put my best efforts forth to the best of my
abilities.

I acknowledge that I have thoroughly read and fully understand the above statement and agree to
follow them by signing this form.
