AFFIDAVIT

I, AIKO M. SINAG, of legal age, single, Filipino and resident of Brgy. Dacanlao, Calaca, Batangas after
having been duly sworn in accordance with law, hereby depose and state:

1. That, l am a BSIT graduate at STI College Balayan, Inc. located at Ped Plaza Business Centre
Ermita St. Balayan, Batangas.

2. That, I have fully paid my tuition fee for the 1 st and 2nd term of the Academic Year 2021-2022
.
3. That, I am a grantee of CHED TDP-TES financial assistance amounting to Php7,500.00 for the 1 st term
and Php7,500.00 for the 2nd term. The school already received the grants resulting to overpayment of
Php15,000.00 to my account.

4. That I am executing this affidavit to attest to the truthfulness of the above stated facts and likewise to
refund the amount of Php15,000.00 from STI College Balayan, Inc.
