AFFIDAVIT

I, CLARO MARASIGAN MACUHA, of legal age, Filipino, and a

resident of Brgy. Aplaya, Bauan, Batangas, after having been duly sworn
to in accordance with law hereby depose and state:

That I am the father of MAEUS GESU MASANGCAY MACUHA;

That as a requirement for him to apply for a Student Permit to be

issued by the Land Transportation Office, I am required to execute this
Affidavit;

That I am giving my full consent in him applying for a Student

Permit;

That I am fully aware of the effects and consequences of allowing

him to obtain a Student Permit;

That I am executing this affidavit to attest to the truth of all the

foregoing facts, and for other legal purposes this may serve.

IN WITNESS HEREOF, I have hereunto set my hand this 6th day of
