WAIVER OF RIGHTS

KNOW ALL MEN BY THESE PRESENTS;

I, JONATHAN S. LUMANDAZ, of legal age, Filipino, married, and a

resident of San Lorenzo, Buguey, Cagayan, subscribing on oath, depose
and state, That –
1. I was employed by the Department of Education Region 02, Division of
Cagayan on June 5, 2017 up to the present.

2. I am currently holding a position of Teacher II in the Junior High school

Department at Camalaniugan National High school, Camalaniugan,
Cagayan.

3. I requested my transfer from my present place of assignment to Aparri

School of Arts and Trades, Aparri, Cagayan.

4. I hereby voluntarily waive my rights and interests over my present

position and/or rank in consideration of my transfer to my desired place
of assignment;

5. I freely ACCEPT, ACQUIESCE and ASSENT to demotion of my rank

as Teacher II in order to effect my transfer.

6. I hereby RELINQUISHED any claim or demand against my employer

(Department of Education) regarding the demotion of my rank.

7. I hereby affirm that I have voluntarily executed this instrument out of my

own free will, without force, intimidation or violence from any person
present at the time of execution of this presents, nor anything which
somehow may vitiate my consent;

8. I am executing this affidavit with full knowledge of the consequences

thereof, and for any legal purpose this affidavit may serve.
