MEMORANDUM OF AGREEMENT FOR THE FULL IMPLEMENTATION OF ENHANCE SCHOOL

IMPROVEMENT PLAN

SY. 2019-2020

This Memorandum of Agreement is entered into this – th __________, 2019 in Esperanza National High
School Brgy Esperanza Matalom, Leyte, by and between:

The Esperanza National High School, with School Identification Number 303370 a public high school, with
principal address at Brgy. Esperanza Matalom, Leyte, represented in this Agreement by its Head teacher,
Teotima B. Golo, Filipino of Legal age, and hereinafter referred to as the SCHOOL;

-and-

The Parent Teacher Association of Esperanza National High School of Brgy Esperanza Matalom Leyte, of
the Republic of the Philippines, with principal address at Brgy. Esperanza Matalom, Leyte and
Represented in this agreement by its GPTA, President EFREN FRANCISCO, Filipino, of Legal age, hereinafter
referred to as the “GPTA”.

WITNESSETH:

WHEREAS, the Department of Education of the Philippines , hereinafter referred to as “DEPEd” is the
Primary government instrumentality mandated to formulate, implement, and coordinate policies, plans,
programs, and projects in the areas of formal and non-formal basic education; supervise all elementary
and secondary Education institutions, including alternative learning systems, both public and private; and
provide for the establishment and maintenance of a complete, adequate, and integrated system of basic
education relevant to the goals of national development;

WHEREAS, the DEPEd believes that for the effective delivery of junior and senior instruction, there is a
need for strong partnerships among internal and external stakeholder that will provide the school the
necessary resources as the case maybe;

WHEREAS, the Esperanza, National High School will start full implementation of Enhance School
Improvement Plan in School Year 2019-2022, WHEREAS, the SCHOOL is mandated to carry out DEPed’s
objectives for physical environment and academic performance as spelled out above;

WHEREAS, to achieve this objective, the SCHOOL needs strong support of all programs and projects found
in the ESIP to the PTA as stakeholders and partners;

WHEREAS, the PTA shall serve as support group and as a significant partner of the school whose
relationship shall be defined by cooperative and open dialogue to promote the welfare of the learners.
