Sir/Madam:

Greetings of Peace!

I have the honor to inform your office that I am seeking for transfer from Munai National
High School to any schools in Kauswagan or Linamon District for the following reasons:

1. I am a bonafide resident of Purok 4, Libertad, Kauswagan, Lanao del Norte. I lived

here together with my family so it is more convenient for me to work within my
locality; as also mandated in RA No. 8190 or the Localization Law.
2. I have been teaching in Munai National High School since September 21, 2015 for
3 years now.
3. I am wholeheartedly that I am working harmoniously within my students, the
faculty and the community of the schools. However, last September 2017 I received
a light threat which prompted me to get worried and felt scared.

For these reason ages, I am humbly appealing to your office to consider my intention of
transfer. Attached herewith is my endorsement letter, proof of residency and other
documents.
