WAIVER

I, GERALDINE T. RECALDE, of legal age, single, Filipino citizen, with

residence and postal address at B3 Nile St., River View Village Bacaca, Davao City,
after having been duly sworn to in accordance with law hereby depose and say:

1. That I am presently employed as Teacher I LSB at Davao City National High

School (DCNHS), F. Torres St., Davao City;

2. That I applied for the Senior High School at Bernardo D. Carpio National
High School

3. That I have submitted the documents required and waiting for the ROV

4. That I am executing this affidavit of waiver to attest to the truth of the

foregoing facts and to attest to the validity and authenticity of the
documents submitted in relation hereof and for whatever legal purpose it
may serve.

and postal address at San Lorenzo, Buguey, Cagayan, after having been duly sworn
to in accordance with law hereby depose and say:

1. That I am presently employed as Teacher I at Camalaniugan National High

School (CNHS), Dugo, Camalaniugan, Cagayan;

2. That I applied for the reclassification of my position from Teacher I to

Teacher III in pursuant to the position classification and compensation
scheme for teaching positions as promulgated by the Department of
Education (DepEd) and Department of Budget and Management (DBM);

3. That I have submitted the documents required for the implementation of

approved Equivalent Record Form (ERF) to the Division Office of Cagayan,
Carig Sur, Tuguegarao City, Cagayan;

4. That as a condition to my application, I will not seek any promotion due to a

natural vacancy pending my application for reclassification;

5. That I am executing this affidavit of waiver to attest to the truth of the

foregoing facts and to attest to the validity and authenticity of the
documents submitted in relation hereof and for whatever legal purpose it
may serve.
