CBT Session 1 - Initial Assessment

Agenda

 Introduce yourself
 Discussion around what CBT entails (keep it simple, even use diagram of 5 factor model)
 Discussion around Confidentiality and Ethics
 Confirm if client consents to share information i.e. student portfolio & voice recordings
 CORE-OM (sometimes called CORE-34) = measure psychological distress
 GAD-7 = measure anxiety levels
 PHQ-9 = measure depression levels

Questions

What brought you to therapy? (This is when you encourage client to open up – active listening)

What would you like to get out of therapy? (Might get idea on rough goals but can be agreed in
subsequent sessions)

Do you have any health conditions? Are you on any medications?

What was it like growing up for you? (Childhood years)

What’s your current sleeping pattern like? (Falling asleep, staying asleep, and rising in morning)

What’s your current job?

Do you have any hobbies?

Do you have a support network? (Family, Friends)

Is there anything that you have stopped doing that you used to enjoy?
