The nurse is planning care for the child with chronic


renal failure.
1. If the nursing diagnosis “Risk for Injury related to
accumulated electrolytes and waste products” was
chosen, what would be an appropriate patient goal?

-The patient will keep their electrolyte levels within


normal range, and the child won't show any signs of
waste product excessive accumulation.

2. What are four appropriate nursing interventions


for this nursing diagnosis?
Assess the electrolyte panel, creatinine, and
I&O.
Keep an eye out for symptoms of fluid
overload, such as decreased oxygen saturation,
lung crackles, pitting edema, swollen neck
veins, and rapid bounding heartbeat.
Serum osmolality, electrolytes, bicarbonate,
pH, serum calcium, CBC, and urinalysis levels
should all be monitored.
Observe patient compliance with medication,
nutritional advice, and fluid restriction.
