Research Title : Post-stroke cognitive impairment: Nurses’ roles in the
assessment and management in Indonesia

Hereby, we kindly ask your permission for our student to conduct a quantitative online survey as part
of their PhD studies, with nurses who are involved in delivering community-based stroke care to stroke
survivors in your working area (UPT Puskesmas Singkawang Tengah, Kalimantan Barat). The survey
asks nurses about their experiences and perceptions in relation to assessing and managing post stroke
cognitive impairment in their day to day practice. Ethical approval to undertake the survey has been
sought from ethics committee of the College of Medical, Veterinary and Life Sciences, University of
Glasgow.
