Greetings in the Lord!

We would like to inform you that the Common Study on Prayer is slated on September 23-25,
2018 at the Minor Basilica of Our Lady of the Rosary of Manaoag in Pangasinan. In this
regard, we would like to get the following information from you:

1. Names of the Participants (Formators and Formandees)

2. Nicknames of the Participants (For nametags)
3. T-Shirt sizes

The participants are expected to arrive in Manaoag on September 23, 2018 at 4:30 PM. Vehicles
will be provided to pick up the participants at Sto. Domingo Church, Quezon City on the same
day at 1:00 PM. Should you wish to avail of this transportation, please indicate your
confirmation together with the other requested details.
