REQUEST FOR REFUND

This letter intends to request for refund for Mr. Bo Sanchez for the event of Truly Rich club
held at Dome 2 last October 27-30, 2022 for withholding tax. The amount P3,388.56 which is
the withholding tax should be refunded since the have presented the Tax Certificate. The
mode of payment for this request is bank transfer. We have appended the attachments to
support this request.
