HISTORY OF PRESENT ILLNESS:

The patient started experience Urinary Tract Infection with onset fever and chill. The doctor

given her antibiotic good for 7 days but she did not continue to consumed the prescribed medication

because she felt that she is already okay and she doesn’t need to take those medicine. After a couple

of months she admitted to hospital because of severe pain located at the back. Upon assessment and

laboratory, the doctor diagnosed her Chronic Glomerulonephritis.

PAST HEALTH HISTORY:

The patient stated that she doesn’t have any history of illness. She also stated that she didn’t

experience hospital admission before. She said that in their family, her mother side has a history of

renal diseases.
