AFFIDAVIT OF LOSS

I, DANIEL M. CORDOVA JR., of legal age, married, and a resident of Villarica,

Midsayap, Province of Cotabato after having been sworn to in accordance with law do
hereby depose and state, that:

1. I am a holder of a Passbook together with the Armed Forces and Police

Savings and Loans Association” (“AFPSLAI”); AFP Finance Center Multi-Purpose
Cooperative (“AFPFC”); Philippine Army Finance Center Producers (“PAFCIPIC”);
Agricultural Cooperative Development, Inc. (“ACDI”); and my Vaccination Card;

2. However, all the above-mentioned passbooks were lost or misplaced. I

remembered having kept the said passbooks inside my mini bag. However, the said
mini bag must have left while I was on duty in Davao City on July 12, 2022 around
10:00 AM;

3. Despite diligent search and efforts to locate them, I could not find the
same such that I believe that they are now lost beyond recovery;

4. As such, I am executing this Affidavit for the purpose of re-issuance of my

Passbook with the Armed Forces and Police Savings and Loans Association”
(“AFPSLAI”); AFP Finance Center Multi-Purpose Cooperative (“AFPFC”); Philippine
Army Finance Center Producers (“PAFCIPIC”); Agricultural Cooperative Development,
Inc. (“ACDI”); and my Vaccination Card, as well as other identification cards issued to
me by the Armed Forces of the Philippines, to attest to the truth of the foregoing, and for
whatever legal purposes it may serve.
