    I Hereby declare that :

    1. MT. JI TAI has NOT visited Countries with local transmission of EBOLA Influenza as
    indicated by WHO in the 2 days.
    2. Our crew members are NOT suffering from the following symptoms:
    Fever with severe headache or muscle pain, vomiting or diarrhea, abdominal pain or
    unexplained bleeding.
    3. No person died onboard of any unexplained reason / disease
