    NUR 098 - HEALTH ASSESSMENT RLE

    HEALTH HISTORY

    Guidelines:

    1. The student performs and records his interview for 3 to 5 mins only.
    2. Instruct students to interview at least one person living in the same household.
    3. Observe proper decorum in interviews and note areas where students can improve.
    4. After the interview, evaluation of students occurs by means of endorsement of details to the instructor
    during their face to face classes.
    5. Total score of 100 deducted to areas where the student misses out a detail.
