last 4th of July, 2022 after I graduated from Senior High School and also told I can retrieve it
once my classes at Rizal Technological University starts.
I am currently not able to do it myself as I am currently admitted with Dengue and I need it for
a Quezon City Scholarship to be passed this 25th of October.

To support my claim, I have appended two of my identity cards, respectively:

1) POSTAL ID
2) TIN ID

to serve as a proof that I allowed the bearer of this letter to claim the document on my behalf.
