QUEZON CITY SCHOLARSHIP PROGRAM

SCHOLARSHIP CONTRACT

I, AMILHUSSEIN RENOMERON MORALES with residence at 017 OBANIC STREET,

COMMONWEALTH, QUEZON CITY and studying at RIZAL TECHNOLOGICAL UNIVERSITY
after having been informed of the scholarship guidelines and administrative procedures of the
Quezon City Scholarship Program thru the Quezon City Youth Development Office (QCYDO),
hereby agree to the following:

I am a beneficiary of the Quezon City Scholarship Program under the SCHOLARSHIP FOR
TERTIARY STUDENTS CATEGORY;

I am entitled to a scholarship grant not exceeding TEN THOUSAND PESOS (Php

10,000.00) for tuition fee and FOUR THOUSAND PESOS (Php 4,000.00) for stipend per
school year for a period of (4) FOUR years, excluding approved Leaves of Absences (LOAs);

I shall claim the scholarship grant following the schedules and deadlines set by the QCYDO,
in coordination with the City Treasurer's Office. If not claimed on or before the given deadline, the
grant shall be revoked and returned to City government funds;

I shall follow the conditions stated in the scholarship guidelines, and that any violation of
such would be a ground for the termination of the scholarship. General grounds for terminations
include: (1) non-fulfillment of conditions stated in the Scholarship Guidelines; (2) forging,
falsification, and misrepresentation; (3) conviction of a crime and health conditions that render
the scholar unfit to pursue studies during the term/ school year applied for, and; (4) other causes
as may be determined by the Youth Development Office

I hereby affirm that I have read and understood this Contract and shall abide by the terms herein.

QUEZON CITY SCHOLARSHIP PROGRAM

PROGRAM DESCRIPTION

Ang Quezon City Scholarship Program ay may mandatong tumugon sa pangangailang pang-
edukasyon ng mga karapat-dapat na mag-aaral, lalo na ang mahihirap. Ang programa ay
may apat na scholarship categories: (1) scholarship for senior high school students; (2)
scholarship for tertiary students; (3) scholarship for masters and doctorate students, at; (4)
scholarship for vocational courses students.

Ang scholarship ay saklaw lamang ang bilang ng taon o school term ayon sa
curriculum/program/degree requirements ng iskolar, at kinakailangang i-renew bawat
school year o school term.

BENEFITS

Simula School Year 2021-2022, ang mga iskolar ay makakatanggap ng sumusunod na grant
at stipend bawat school year:

*Isang beses lamang makakatanggap ng thesis/dissertation grant and iskolar. Ito ay
ipagkakaloob sa iskolar matapos maaprubahan ng QC YDO ang policy/program proposal na
nakapaloob sa thesis/dissertation ng iskolar.
**Ang scholarship grant para sa vocational courses students ay binibigay ng isang beses
lamang. Ang scholarship ay matatapos kapag nakumpleto na ng iskolar ang itinakdang bilang
na oras ng pagsasanay.

ELIGIBILITY

Para maging eligible sa programa, ang aplikante ay kinakailangang: (1) residente ng Lungsod
Quezon; (2) hindi tumatanggap ng scholarship mula sa ibang LGU, at; (3) naka-enroll o naka-
rehistro sa isang kinikilalang educational institution sa panahon ng pag-apply ng
scholarship. Kinakailangan ding makamit ng aplikante ang mga naka-saad na qualifications
sa susunod na bahagi.

QUALIFICATIONS

I. SCHOLARSHIP FOR SENIOR HIGH SCHOOL STUDENTS

1. Nagtapos sa pribado o pampublikong sekondaryang paaralan sa Lungsod Quezon
2. Nagtapos ng Grade 10 na kasama sa pangkalahatang Rank 1 hanggang 10
3. May kabuuang grado o General Weighted Average na di bababa sa 85%

II. SCHOLARSHIP FOR TERTIARY

STUDENTS Academic Scholarship

1. Nagtapos sa pribado o pampublikong sekondaryang paaralan sa Lungsod Quezon

2. Nagtapos ng senior high school na kasama sa pangkalahatang Rank 1 hanggang 10
3. May kabuuang grado o General Weighted Average na di bababa sa 1.5 o katumbas
nito para sa mga pangkalahatang Rank 1-2, at di bababa sa 1.75 o katumbas nito
para sa mga pangkalahatang Rank 3-10

Economic Scholarship

1. Nagtapos sa pribado o pampublikong mataas na paaralan sa Lungsod Quezon

2. May kabuuang grado o General Weighted Average na di bababa sa 3.0 o katumbas
nito
3. Kabilang sa mga sumusunod na grupo: indigent families, displaced/relocated
families, and vulnerable or marginalized sectors (PWDs, Kasambahays, ALS
graduates, solo parents, children in conflict with the law, families of tricycle
drivers and operators)

Athletic and Arts Scholarship

1. Nagtapos sa pribado o pampublikong mataas na paaralan sa Lungsod Quezon
2. Nakatanggap ng top individual award/recognition para sa isports at sining sa
kasalukuyang school year/school term/playing season na bigay ng kilalang
institusyon
3. May kabuuang grado o General Weighted Average na di bababa sa 2.5 o katumbas
nito

Youth Leaders Scholarship

1. Nagtapos sa pribado o pampublikong mataas na paaralan sa Lungsod Quezon
2. Nakatanggap ng anumang leadership award mula sa lungsod
3. Kabilang sa Konseho ng Sangguniang Kabataan o Supreme Student Council o
naninilbihan bilang presidente/bise-presidente (o katumbas) ng rehistradong
organisasyong pangkabataan sa Lungsod Quezon
4. May kabuuang grado o General Weighted Average na hindi bababa sa 2.5 o
katumbas nito

Specialized Courses Scholarship

1. Nagtapos sa pribado o pampublikong mataas na paaralan sa Lungsod Quezon
2. May kabuuang grado o General Weighted Average na di bababa sa 1.75 o
katumbas nito
3. Naka-enroll o naka-rehistro sa anumang CHED priority courses o specializations,
subalit prayoridad ang mga courses o specializations na kasalukuyang
kinakailangan ng Lungsod Quezon. Ang “List of Priority Courses” ay i-uupdate
bago magsimula ang school year at ipo-post sa

III. SCHOLARSHIP FOR MASTERS AND DOCTORATE STUDENTS

1. Empleyado ng lokal na pamahalaan ng Lungsod Quezon
2. Naka-enroll o naka-rehistro sa isang kinikilalang Higher Education Institution o
alinmang kinikilalang International Institution
3. Nagtatrabaho ng hindi bababa sa dalawang taon at may posisyon na may katumbas o
hindi bababa sa Salary Grade Level 14
4. May kabuuang grado o General Weighted Average na hindi bababa sa 2.0 o katumbas
nito.

IV. SCHOLARSHIP FOR VOCATIONAL COURSES STUDENTS

1. Kabilang sa mga sumusunod na grupo: indigent families, displaced/relocated families,
and vulnerable or marginalized sectors (PWDs, Kasambahays, ALS graduates, solo
parents, children in conflict with the law, families of tricycle drivers and operators)
2. Naka-enroll o naka-rehistro sa alinmang TESDA-accredited institution o iba pang
institusyon ng pagsasanay na kinikilala ng pamahalaan ng Lungsod Quezon.
3. Naka-enroll o naka-rehistro sa kursong bokasyonal na naglalayong paghusayin ang
kasanayan na may kaugnayan sa:
a. Service and Logistics
b. E-commerce
c. Business Start-up / Business Administration
d. Financial Literacy
e. Information and Communications Technology
f. Product/Service Development
g. Office Administration / Business Communication

SCHOLARSHIP APPLICATION PROCEDURE

Scholarship Application (For New Scholars)

1. Kumuha ng QCitizen ID at i-rehistro ang email address sa QCeServices.
2. Mag-apply para sa scholarship online.
a. Pumunta sa https://qceservices.quezoncity.gov.ph/
b. Mag log-in gamit ang email na naka-rehistro sa QCeServices
c. Piliin mula sa listahan ng services ang “QC Scholars”
d. I-fill-out ang online Scholarship Application Form
e. I-upload ang scanned copy ng mga sumusunod na documento:
i. True copy of grades/Transcript of Records/Form 137 or 138 for the
previous school term/school year
ii. Any of the following IDs:

aplayan. Ang mga nasabing documento ay i-uupload gamit ang QCeServices.

4. Makakatanggap ng notification na na-aprubahan ang application for scholarship at

maari nang i-generate ang Scholarship Contract at Certificate of Scholarship gamit
ang QCeServices.
5. Makakatanggap ng notification na maaari nang i-claim ang scholarship grant.

Scholarship Renewal (For Existing Scholars)

1. Mag-renew ng scholarship online.

b. Mag log-in gamit ang email na naka-rehistro sa QCeServices
c. Piliin mula sa listahan ng services ang “QC Scholars”
d. I-fill-out and online Scholarship Renewal Form
e. I-upload ang scanned copy ng mga sumusunod na documento:
i. True copy of grades/Transcript of Records/Form 137 or 138 for
the previous school term/school year
ii. Proof of school enrollment/registration/acceptance for the
current school year/school term
iii. Approved Leave of Absence (para sa mga iskolar na hindi nag-enrol /
hindi nakipag transaksyon sa nakaraang school year o school term)
2. Makakatanggap ng notification para sa interview.
3. Makakatanggap ng notification na na-aprubahan ang application for scholarship at
maari nang i-generate ang Certificate of Scholarship gamit ang QCeServices.
4. Makakatanggap ng notification na maaari nang i-claim ang scholarship grant.
