1) Application form/filing fee

2) Copy of DENR approved plan
3) Copy of technical description (BL Form V-37)
4) Simplified sketch plan
5) Certificate of no pending land registration proceedings covering the land apllied for from
the Regional Trial Court (RTC)
6) Local Government Unit (LGU) zoning ordinance showing that the land subject of the
application is zoned as residential area
7) Certification that the land applied for is not needed for public use or purpose from the
LGU-MPDO
8) Updated Tax Declaration
9) Certificate of Tax Payments
10) Deed of Conveyance (Affidavit of Waiver, Transfer of Rights, Deed of Sale, Donation)
11) Barangay certification where the land is located
12) Certification that the land is within Alienable and Disposable (A&D)
13) Certificate of Notice of Posting of Application
14) Investigation and Ocular Inspection Report
15) Order of Issuance of residential Free Patent
16) Accomplished Judicial Form
17) Transmittal Letter to PENRO
18) Others: SPA as the case may be
