DISCHARGE PLAN

Objectives:

After an intervention of health education and discussion about the discharge plan, the
client will be able to: 
 Develop knowledge regarding to the complications of having an ovarian cyts .
 Integrate knowledge on how to prevent sudden complications that will lead to severe
cases like cancer.
 Know the importance of following the doctor's order (instructions).
 Verbalize understanding of her condition, complications, and medical condition.

NURSING INTERVENTIONS

Medication:
Encourage the client to take prescribed medications. Educate her when it comes to the
appropriate time of taking them, and it's correct dosage.
RATIONALE

Taking the right medication (with the right time, route, and dosage) will alleviate pain, and will
help with the fast recovery of the patient.
Management:
Encourage client to avoid vigorous activity that can lead to cause ovarian torsion.

RATIONALE

To avoid vigorous activity until the cyst grows smaller and goes away. Extreme activity might
cause ovarian torsion.

Treatment:
A surgical procedure called laparoscopy is the most efficient method for treating infertility
related to endometriosis, and the procedure seems to be the most effective particularly at stage III
rAFS.

LAPAROSCOPIC
a surgical procedure in which a fiber-optic instrument is inserted through the abdominal wall to
view the organs in the abdomen or to permit a surgical procedure.

RATIONALE
Laparoscopy relieves pain,can be used to help diagnose a wide range of conditions that develop
inside the abdomen or pelvis. It can also be used to carry out surgical procedures, such as
removing a damaged or diseased organ, or removing a tissue sample for further testing (biopsy).

HEALTH TEACHING
Promotive:
To promote optimum level of awareness and knowledge about ovarian cyts
Preventive:
There's no way to prevent most ovarian cysts. But, regular pelvic exams help ensure that changes
in your ovaries are diagnosed as early as possible. Be alert to changes in your monthly cycle.
Make a note of unusual menstrual symptoms, especially ones that go on for more than a few
cycles.

Curative :
Encourage client take medicine as directed. Follow doctor's instructions about activity during
healing process.

RATIONALE

Following the doctor's order or instructions while taking your prescribed medication for pain,
etc. would help in a faster recovery.

Rehabilitative:
Encourage client to engage in a Physical therapy to rebuild strength, flexibility.

RATIONALE

Physical therapy provides a variety of benefits, including reduced pain, improved function and
more that would be beneficial to someone who had laparoscopy.

Out Patient:
Follow up an appointment as directed by your doctor.

RATIONALE

It helps the health care practioner to montior the clients condition.

Diet:
Instruct client to eat various healthy foods like fruits and vegetables . Foods that are rich in fiber
are pears, oranges, lentils, peas. Including broccoli, greens, almonds, berries, and squash. lean
proteins, including fish, tofu, and chicken. anti-inflammatory foods and spices, including
tomatoes, turmeric, kale, olive oil, and almonds.
RATIONALE

Proper deit contain phytochemicals that stop the re-absorption of estrogen hormone in the body.
They further reduce the risk of ovarian cancer.

EVALUATION
