I am a 2016 MSU-GenSan alumna who currently lives and works in Davao del Norte.
I intend to advance my career by studying more. However, due to geographical
distance, I am unable to personally process the documents I require.

Prior to this, I requested and received my documents (Honorable Dismissal & Good
Moral), with the exception of the Transcript of Records (TOR), which requires a
requisition form from the institution where I am currently enrolled. Hence, I am
delegating authority to Ms. Erica Deb A. Manginsay to process it on my behalf.
