WAIVER AND CONSENT

I, Arvin Jimenez Oliveros, hereby knowingly and freely declare, undertake, and warrant that:

I voluntarily agree to the specimen collection and testing for COVID-19 of FIRSTAIDE DIAGNOSTICS and its partner
facilities through a nasopharyngeal and/or oropharyngeal swab.

I understand that, as with any medical test, there are certain inherent risks associated with the specimen collection,
which while generally does not hurt, may cause discomfort or injury. I also understand that there may be unforeseen
incidents that may arise in the course of the testing that is beyond the control of FIRSTAIDE DIAGNOSTIC even with the
care undertaken by it thru its personnel like but not limited to swab stick may break and be lodged in my person.

I understand that, as with any medical test, there is the potential for a false positive or false negative COVID-19 test
result.

I authorize FIRSTAIDE DIAGNOSTICS and its partner facilities to disclose my test results to my employer/principal,
Grace marine and shipping corp., as well as to the authorities as per RA 11332, its implementing rules and
regulations, and such other related laws, rules and/or ordinances.

I hereby waive any and all rights,claims, or causes of action of any kind whatsoever arising out of my participation in
this specimen collection, and do hereby release and forever discharge FIRSTAIDE DIAGNOSTICS, including its officers,
directors, employees, agents and representatives from any and all injury I may suffer as a direct or indirect result of
the specimen collection. I have been informed of the purpose of the specimen collection and possible risks, and I have
received a copy of this Informed Consent.
