Patient Referral Policy

POLICY:
This policy is to ensure that patient is referred to another centre only if the required service is not
available at Live’ly ®. There should be proper justification for referral.

PURPOSE:
The purpose of this policy is to ensure the availability of basic services at Live’ly ® and to protect the
client’s rights to avail appropriate service for which she/he needs.

REFERAL FORM DETAILS:

-Patient referral type
-Patient information
-Reason for referral
-Significant physical and other findings
-Patient’s condition/status
-Follow up instruction/information
-Receiving facility information
-Referred facility information
-Form should be signed by the dietitian or medical director
