Customer Assessment Policy

(A) Policy Statement

Provides the dietitian with the diet background, dietary motives, current
eating and exercise habits, as well as the medical history of the client. It is
used as a reference for setting the client’s calories and weight goals for
future follow ups. It also helps indicate what lifestyle habits the client needs
to change in order to reach and maintain a healthy body weight

(B) Purpose of Policy


To ensure that Live’ly ® provides nutritious, high-quality food of sufficient
variety to meet the needs and expectations of patients through the customer’s
nutritional assessment.

(C) Procedure

1. During the first consultation, the clinical dietitian will complete the
customer’s assessment sheet by asking the necessary questions in order to
evaluate the client’s dietary status. General time required to fill form:
between 15-45 minutes (depending upon the length of the consultation).

2. Data collected by Live’ly ® includes:

-Date of last blood test


-Medical and Family History
-Medications, Vitamins, and Supplements taken by the client
-Food allergies
-Physical Activity status
-24 hr. recall
-Client’s motives
-Daily beverage intake
-Client’s habits
3. Clients are usually asked for a routinely 2-week follow up depending on the
Dietitian’s decision

(D) Remarks

1. The customer assessment sheet should be completed and kept in the client’s
file for future references.

2. In case the client discontinued the meal plan and re-joined after a year, a new
customer assessment sheet should be done.
