NUTRITION CARE PROCESS (ADIME PROCESS)
Introduction
Nutritional care for the ill or hospitalized patients includes an assessment of the adequacy of
nutritional intake, manipulation of the diet when necessary, provision of enteral or parenteral support when
appropriate, and intervention in the form of counseling or education when needed. The Nutrition Care
Process is the process of planning and meeting the nutritional needs of a client. It is designed to improve the
consistency and quality of individualized car for patients/clients or groups and the predictability of the
patient/client outcomes.
Valid health care is based on individual care
Comprehensive health care is best provided by a team of various health professionals and support
staff persons.
A personalized health care plan, based on identified individual needs and goals, guides, actions to
promote healing and health.
Objectives
At the end of this lesson, you can:
1. Assess the nutritional status of a given client using relevant parameters and appropriate nutritional
assessment tools.
2. Formulate with the client relevant nutrition diagnosis.
3. Manage resources efficiently and effectively.
4. Implement safe and quality interventions with the client to address the nutritional problems and
issues.
5. Provide health education in nutrition and diet therapy to targeted clientele (individuals, family,
population group or community).
6. Document nutritional status accurately and comprehensively.
7. Explain the responsibilities of the dietary and nursing department in patient care.

NCP: Acute Care
Nutrition Assessment
Medical History: 72 y.o. female admitted with decompensated CHF; heart failure team consulted; has been admitted
with same dx x 2 in past month; meds: Lasix and Toprol; current diet order: 2 gram sodium; has lost 5 pounds in 24
hours since admission; Output > input by 2 liters
Nutrition History: has been told to weigh self daily but has no scale at home. Does not add salt to foods at the table.
Noticed swollen face and extremities on day prior to admission. Day before admission ate canned soup for lunch and
3 slices of pizza for dinner; does not restrict fluids; has never received nutrition counseling.
