In order to promote, maintain and provide assurance of high standards in health, safety, quality and
the environmental aspects of our business we would be grateful if you could supply the following
information and any relevant supporting documentation.

In line with the company QHSE policies and standards, it has become the mandate for us to ensure all
contractors/vendors supplying products and services to the company to have performed the pre-qualification
QHSE assessment.

This effort comes with the objectives to ensure personal safety as well as product integrity as building on the
Company culture and credo of “Safe Product, Safe Operation, Safe Person & Community”.

Each year, the Company rewards and recognizes contractors who have achieved substantial improvement in
QHSE performance such as accident record, safe operation or quality delivery.
