    Do not "tick". Write OK or use the number of the specific deviation given in the legend below. If the equipment is defective it
    must be tagged “Defective” and must be reported to the person responsible for the repair of the equipment. If the equipment
    is beyond repair it must be destroyed and discarded.

    LEGEND

    1. Loose rungs (move by hand). 9. Defective centre guide for extension.


    2. Loose nails, screws, bolts, etc. 10. Defective extension locks.
    3. Loose mounting brackets. etc. 11. Defective rope pulley.
    4. Cracked, broken, split or bent stays/rungs. 12. Deterioration of rope.
    13. Painting of wooden ladders needed.

    5. Splinters on stays or rungs 14. No identification number.

    6. Damaged/worn non-slip devices 15. Storage place not acceptable.

    7. Unstable 16. Other – specify.

    8. Loose or defective hinge spreaders/hinges.
