I. What is a Medication Card?

● Medicine cards are flashcards that provide information on each medication for
nursing students
● Medication cards make learning about medications easier, faster, and more
memorable.
● It is an accurate and current list of medications that helps your medical
professionals keep you safe by being aware of all your medicines, vitamins, or
supplements.

II. Different Uses of Medication Cards

● Medication cards can help you, your doctors and family members keep tabs on
prescriptions, over-the-counter medicines, vitamins and supplements you take.
● This medication card is used to maintain a current record of any prescriptions of
the patients.

III. Different Types of Medication Card

● Personal Medication Cards

❖ This type of medication card is used in order to personally keep track of the list of
medication that the patient is required to take.
❖ This card can be taken at home by the patients.

Wallet Type Medication Card

● This medication card includes a medication list, health condition, past surgeries, allergy
information, and questions that patients should ask their doctor and pharmacists. The
format is designed to be folded to fit into a plastic sleeve or in a wallet.
● The card’s easy-to-carry design is handy in case of an emergency.

Hospital Medication Card


● Hospital medication card includes the hospital information, name of patient, age, sex,
address, date, attending physician’s name, ward name/location/number, bed number,
name and amount of drug ordered including its dosage, frequency, route of
administration, the name and signature of the pharmacist who prepared the medication/s
and the medicating nurse.
● Hospital medication card is an important instrument in order to keep the pharmacist,
physician and nurses in track with the medications of the patient.

IV. Benefits of Medication Card
● Medication cards help improve medication administration for nurses and patients.
● It helps simplify prescription administration and patient information.
● It Easy to organize and disseminate the drug name, route of administration, dosage,
frequency, date and time of administration and expiration date for administering the
medication, if appropriate. Rationale: Orders for certain medication (Narcotics,
antibiotics) expire after a specified time frame and they need to be reordered by the
primary care provider.
● Easy to notice if dose is missed or mismatched. This Helps compare the label of
medication container or unit-dose-package. This is a safety check to ensure that the right
medication is given.

V. Background of medication card

Initially developed by Iowa Healthcare Collaborative, St. Luke’s Hospital and Mercy
Hospital and the Iowa Pharmacy Association.

- Medication card was made by Iowa Healthcare Collaborative , St.Luke’s Hospital


and Mercy Hospital to show all the patient’s medication on a regular basis.
- Beginning January 2019, Pharmacy and nursing personnel distributed the
medication cards to patients who were initiated on any new medications and
anyone who was thought to have potential benefit from medication education.
Patient education using the cards was documented in the electronic medical
record, and data was collected on the number of medication cards distributed
weekly and the percentage of patients who were discharged with at least one
medication card.
- Medication cards appear to be a simple and effective tool that can be incorporated
into the daily nursing and pharmacist workflow to ensure that patients are
educated about new medications prior to discharge and improve patient
satisfaction as indicated by the HCAHPS survey.

● Format
The format of medication cards may vary depending on hospitals or personal preferences
of people, as there are plenty of medication card templates available on the internet. But
below is an example of a hospital medication card from the Perpetual Help Medical
Center - Las Piñas Hospital.
