Qualitative Questionnaire
Dear BEED Students,
We hope this letter finds you in good health and spirits. We are writing to seek your help in conducting
our research on the effectiveness of extension programs through documentary analysis.
The purpose of this research is to determine the benefits and limitations of extension programs for the
community, as well as to identify the factors that have contributed to their success or failure. As a
respondent, your views and experiences will help provide valuable insights that will help us gain a deeper
understanding of how extension programs work and what can be done to improve them.
Participation in this study is voluntary, and all information provided will be kept confidential. The results
of this research will be used to develop strategies and recommendations to improve the delivery and
effectiveness of extension programs in the future.

RA 10173 refers to the Data Privacy Act of 2012, a law in the Philippines that protects the privacy of
personal information of individuals. The law regulates the collection, use, storage, and sharing of personal
information by government agencies and private entities. It also aims to promote the rights of individuals
to their own personal information while ensuring that personal information in the possession of entities
are utilized for legitimate purposes only. The law imposes hefty penalties and/or imprisonment for
individuals and organizations that violate its provisions.
