About SMART Health Cards

A SMART Health Card contains a specific part of your medical record information such as your vaccination
history and COVID-19 test results. It allows you to keep a copy of your records on hand and easily share
this with others as you choose.

Have your SMART Heath Card available to show your vaccine status for school registration or to travel. If
you are a parent or care giver, you can keep SMART Health Cards for yourself or for others, just as you
would any other medical information. For more detail, please visit our COVID-19 Status help page or the

How to share this SMART Health Card?

To allow someone to verify your COVID-19 information, all you have to do is show them the 2D barcode
(QR code) above. You can do this from a printout, from the screen of your smartphone or other digital
device, or by using an app on your phone to send them a digital copy of it.

How to save this SMART Health Card?

To save your SMART Health Card QR code:

1. Take a screenshot of the QR code with your smartphone or other digital device and share
2. On Kp.org, print out the QR code using your web browser's Print function or the Download
Printable PDF button
3. Download the Common Pass app for iOS from the Apple app store or the Common Pass app
for Android from the Google Play store. Once you have installed the app, follow its instructions to
"Create New Pass." You can then either scan the QR code shown above or choose "Digital Health
Certificate" to import the file from the "Download SMART Health Card as a digital file" button on
KP.org
