    This inquiry is for the marks that do not appear on the portal, or ‘NGR’ is recorded, AND when the student
    has fulfilled all course requirements and did the final exam.

    INSTRUCTIONS:

    1. Print the form and fill in all required information with clear and legible writing. One form per each
    exam.
    2. Submit the filled form to Registry.
