In consideration of my enrollment and acceptance at Cansilayan Farm School, I


hereby promise and commit myself to obey the following rules and regulations.
I understand the Disciplinary measures in case of violation of Good Discipline
on Chapter 3 Section 1-4 (D.O No. 92 pages 22-23.)
From my parent or guardian or being to school for a short conference with the
section adviser
1. I shall attend classes regularly and will comply with the reasonable class
requirements. In case of absence or out from classes, I am required a
letter of explanation from my parent or guardian or being with them to
school for a short conference with the section advisers or guidance as the
case maybe.
2. I shall wear the prescribe school uniform and the school identification
card (ID) whenever I am in school. I will inform right away my adviser in
case I lost my ID.
3. I shall observe proper grooming at all times and shall keep myself neat
and clean. In addition, (for male students) I shall observe the acceptable
haircut at least (2) inch above the ear and three (3) inches above collar.
4. I shall help maintain orderliness and cleanliness inside the school
campus.
5. Co-curricular activities shall be subject to prior approval of the school
head especially activities after 6:00 P.M. The school is not liable on any
activities outside the campus not sanctioned by school activities.
6. Meetings, assemblies, convocations and activities shall be held in the
presence and with knowledge of the principal, head teacher or guidance
counselor/designate, teachers/advisers/ club moderator.
7. No student or visitor shall be allowed inside the school campus after the
last class period in the afternoon.

8. That school officials and teachers have the right to impose appropriate
disciplinary measures in case of offences.
9. I will not show any endearment or public display of affection in any
places inside the school premises when I am with my boyfriend or
girlfriend.
10.Suspension /Expulsion (Section 145 0 151, Part VI Chapter III, Service
Manual 1960)
 The first and other offenses which are not very serious in nature, a
suspension from school not to exceed three (3) days may be authorized by
the principal without the prior approval of the Division Superintendent.
However, parents must be informed by the teacher or the school principal
of any misconduct on the part of the student for which disciplinary action
is necessary.
 For persistent offender or one guilty of a serious offense, a suspension for
not more than (1) year may be imposed subject to the approval of the
Division Superintendent.
 Suspension for the school year or more, or expulsion from school can be
only by the Secretary.
 In all cases of suspension, a written promise of future exemplary conduct
signed by the student and countersigned by his/her parents or guardian
shall be required as a condition for readmission and must be required in
the case of suspension for more than three (3) DAYS.
 Offenses punishable by suspension or expulsion depending on the
seriousness of the offenses:

 Gross misconduct
 Gambling of any sort
 Drinking intoxicants and liquor
 Carrying and concealing injury to others
 Extortion or asking money from others
 Fighting, causing injury to others
 Cheating and stealing
 Assaulting a teacher or any other school authority or agents or
students
 Smoking inside the school promises
 Vandalism, writing on or destroying property like chairs, tables,
windows, books laboratory equipment and others.
 Using, possessing and selling of prohibited drugs.
 Hazing in any form or manner whether inside or outside the school
premises such as fraternities, sororities and others.
 Immorality / sexual harassment

 Instigating, leading or participating in concerted activities leading


to stoppage of classes.
 Preventing, threatening students, or faculty members or school
authorities from discharging their duties or from attending classes
or entering school premises.
 Forging or tampering with school records or transfer forms.

In case I violated any of the above – stated rules and regulation, I accept
any voluntary actions which the school authorities will impose on me, which
possible refusal or disqualification for study in this school.

Child Protection School Policies

The Guidance Office administers program, services and activities design to


promote a wholistic approach to personal growth. The guidance program focuses on
self-discovery and uses techniques and approaches through which the individual
potentials, skills and experience will facilitate the optimum actualization of his/her
personhood.

CHILD PROTECTION AND ANTI – BULLYING POLICIES


We believe every student should enjoy our school equally, feeling safe, secure
and accepted regardless of color, race, gender, popularity, athletic ability, intelligence,
religion and nationality. Bullying can be pushing, shoving, hitting, and spitting, as
well as name calling, picking on, making fun of, laughing at and excluding someone.
Bullying causes pain and stress to victims and is never justified or excusable as “kids
being kids”, “just teasing”, or any other rationalization. The victim is never
responsible for being a target of bullying. This school will not tolerate any behavior
that is classified as bullying and will take steps to eliminate such behavior.
One of the programs of the Department of Education is to have a child friendly
school. In order to have this, the school must be free from any violence to the children
and must have a fear free environment. The policies below ensure the safety of the
pupils in our school and community.

1. Information dissemination to the pupils and parents in implementing the


school policies in anti-bullying.

2. Physical Bullying
 First Offense
 The offender will be referred to the adviser and the guidance
counselor/designate for counseling.
 Let the offender signed in the records and agreement made after
counseling.

 Second Offense
 Meeting the parents of the offending child and the victim.
 The victim and offending child together with their parents shall be
referred to the Child Protection Committee for counseling.
 Both parties should affix their signature in the records and agreements
made by the committee.

 Third Offense
 Suspension for 3 days or community service for 3 days supervised by
the Discipline Officer.

3. Psychological Bullying/ Cyberbullying


 First Offense – the offender will sign a promissory note from his/her adviser.
 Second Offense – the offender will be referred to the guidance counselor for
counseling and recording.
 Third Offense – meeting the parents of both parties with their parents and the
members of The Child Protection Committee for counseling.

Students Discipline
The Discipline Advisory Committee, which is compose of the Discipline
Officer, Guidance Designate, SSG adviser and the President of the Teachers’ League
and the School Head as Chairperson of the respondent of the members, shall evaluate
and decide on cases pertaining to violations of the School policies upon admission of
guilt. Upon the determination of the Disciplinary Officer that a case may be
summarily resolved, said case shall be handed by the Disciplinary Advisory
Committee for proper action.

Parents’-Teachers’ Council
Parents’-Teachers’ Council functions as partners of the administrators and staff
of Cansilayan Farm School to establish better communication and effective
cooperation between home and school. It composed of parents and faculty
representative elected during the annual Parents’ General Assembly.

Center for local and external Linkages


The Guidance office coordinate to the external community with information of
the school affairs, hosting arrangement and school planning activities and promotes
local, national and international linkages like The Quidan Kaisahan Partnership with
other institutional and academic organizations.

Group or Individual Conference


To uphold the rights of every individual, with strict compliance of standard
protocol a conference where held at the guidance office depending on the cases arises.

Parents Conference
A letter will be sent to the parents two days prior to the specific time and date
that the adviser wants to meet with the parents and the guidance designate to give just
enough time to both parties to settle the schedule to resolve the concerns that arises.

Career Guidance and Planning


A month-long celebration of Career Guidance is conducted with the
coordination of the advisers of grade 10 for the completing students & Grade 12 for
the graduating students.

Mental and Psychological First Aid


To promote the mental health program set by the department, the SSG, School
Nurse and Guidance designate must work hand in hand to promote a healthy mental
state of every learners as well as the teachers.
Guidance Office must conduct a Psychological First Aid advocacy to parents to
be equipped in preparation to the wellbeing of every individual in times of disaster.

Home Visitations
Advisers who wishes to conduct a home visitation schedule must first file an
anecdotal record, update the guidance designate the current status of the learners and
fill up the specific time and date of the home visitation plan to be signed by the school
head for approval.
Documentation
All pictures of home visitations and conference must be kept confidential and
must properly filed at the guidance office.

School Schedule
Students should be in school by 7:00 AM for a flag ceremony (Monday) and
cleaning of their area of responsibility. At 7:30 AM, all students shall go directly to
their classroom for the first session. Afternoon dismissal at 4:30.
Under no circumstances may Cansilayan Farm School accept responsibility for
students left outside the school before 7:45 AM.
Regular dismissal time is 11:50 AM and 4:40 PM daily. Students are required
to attend flag ceremony every Monday and flag retreat every Friday.
Admissions
All qualified students are admitted in school without discrimination. They are
required to present their live birth certificate or PSA birth certificate and school form
9 or report card during enrolment. Transferees may not be admitted without
document/credential presented during enrolment.

Transfers
If a student into be transferred to another school, the school must be notified at least
two weeks prior to the transfer.

Tardiness
 First Offense – the offender will fill in the admission slip and pass to
the Discipline Officer
 Second Offense – the offender will fill in the admission slip and will
have a conference with the Discipline Officer or the Guidance
Counselor/Designate with community service.
 Third Offense - the offender will fill in the admission slip and will have
a conference with the Discipline Officer or the Guidance
Counselor/Designate together with the parent/s.

Attendance
Regularity of attendance and punctuality are important in the academic
progress of the child and in His or Her character training.
The school list the following legal absences:
 Sickness of a student
 Sickness or death in the family
 Impossible roads or weather which make travel unsafe
 Required presence in court
 Participation in sports or other extra co-curricular activities

All other absences are classified as unlawful detention on the part of the parent
or truancy on part of the child. Teachers are not able to anticipate neither absence nor
can they practically make up work with the student and the student are held
accountable for all missed work to pass the subject.
A written excuse letter from the parent of an absent student must be presented
to the teacher when the student returns to school. These become part of the attendance
file.
No students may leave school during the class hours unless accompanied by a
parent or guardian who must report to the school office. In case of a student’s illness
while in school, the school will call the attention of the elder sibling in school and
allow them to go home, but if the nature of the emergency requires immediate medical
attention, the teacher will call the parent and the Barangay Health Center service. The
parent or guardian must sign the “Permit to Leave the Campus Slip” before the
student leaves the school. This record will be kept on file and is considered a legal
document.

Use of Cellphones
The school strictly imposes a ban on the use of cellphones by the students
during class hours (DECS Orders: Nos. 26, s. 2000 and 70, s. 1999). If a student
caught using the cellphone during class hours without permission of the teacher,
 First Offense – written warning
 Second Offense – the cellphone is confiscated and returned only if the
student will write a promissory note-a written and signed promised that
he/she will not do it again.
 Third Offense – the cellphone is confiscated and returned only to the
parent.

Greeting or “Ang Magalang, Bow”


A gesture that must be made by the students when greeting the visitor/teacher
at the start of the class, end of the class, or bumped with inside the school campus.

“Ang Magalang, Bow” (DO 88, S. 1991) is done by slightly tilting the head
downward in front of the person being greeted and putting both hands together below
the breast.
