RULES AND REGULATIONS FOR TEAM BUILDING

A TEAM BUILDING is thrilling experience where you can develop harmonious relationships,
teamwork, and unity among your classmates. You get the chance to express your opinions and
grow personally. Team Building has established some significant norms and regulations as a
result of these variations and for safety. In addition to making your activity more enjoyable,
following these rules will make it safer for you.

AREA BEHAVIOR. Show respect for the space. Maintain order while using it. You are liable
for any and all damages that you create. Be mindful of the surroundings sounds. No participants
or students are allowed to (1) leave the area for any reason or (2) use other facilities unless they
have the officers’, or heads’ authorization.

LOST ITEMS OR STOLEN ITEMS. You are responsible for your own belongings. Keep all
valuables with you at all times. Report any missing items directly to your officers/heads in which
you think the items was lost or stolen.

STAYING TOGETHER. You must remain in close proximity to one another. No one is allowed
to go alone for any purpose. The most crucial thing is that your group leader always knows
where you are. You are no longer part of the group, never forget that. Be punctual for all
scheduled activities and departure. Keeping people waiting is unjust, and you don’t want to miss
anything! The group leader should always verify attendance both before and after the activity.

ALCOHOL OR CONTROLLED SUBSTANCES. Students possessing or using alcohol or any

narcotic are prohibited. No weapons of any sort are allowed.

If the student should violate any of these rules and regulations, He/she may send home at the sole
discretion of the heads, in such cases, the parent/guardians will be contacted and the student sent
home at own expense.

STUDENT CONTRACT

I have read and understand the TEAM BUILDING Activity's behavior guidelines. I shall comply
with all of these rules and regulations to the best of my ability.

PARENT CONTRACT

I have read, comprehended, and given my support to the guidelines for the team-building
activity. I hereby acknowledge that my child or ward has read the rules and will try his/her best
to abide by them all. It is assumed that signing his behavior contract with one parent or guardian
implies that the other has given their approval.

PARENTS PERMIT AND WAIVER

TO WHOM IT MAY CONCERN:

This is to certify that I/We, the parent/guardian, have given the full consent to my/our
son/daughter _____________________________________ to attend the TEAM BUILDING
2023 at University of Caloocan City - Camarin Campus on October 19, 2023.

This is to further certify that the PASOA Organization, specifically the University of
Caloocan City, as well as its officials and/or duly authorized preventives, shall not be held liable
for any untoward incident beyond control that may happen to my/our son/daughter in connection
with the aforementioned undertaking.
