Program Details
Receive mattress covers and pillowcases that help eliminate dust
Asthmatics Assistance mites for people with asthma.
Free registration fee at participating Boys and Girls Club of
America for members between the ages of 5 and 18 years. Present
Boys and Girls Club Membership
your United Healthcare ID card at the time of registration. Call
member services at 1.844.752.9434 for participating clubs.
Mobile phone government assistance program. Limited to one
Cell Phone discount per household. To apply and choose the program best

Cover chiropractic services for Medicaid members under age 21
Chiropractic Services when medically necessary. Members over age 21 can get up to
six visits a year with an in-network chiropractor.
Covers medical equipment ordered by your doctor or case
Durable Medical Equipment manager. This can include supplies like breast pumps,
orthotics, wheelchairs or walkers.
Free classes on diabetes, nutrition and more to help improve
Health Education
your health. You can even get a ride, to and from the class, at
no cost.

Program Details
Healthy First Steps Helps keep you and your baby healthy during your pregnancy and
your baby's first 15 months of life. Assists with choosing a
pregnancy provider and pediatrician, schedule visits/exams and
arrange rides for your visits, getting supplies including breast
pumps, connect with community resources, etc. Members can also
earn rewards for attending visits.
After surgery or a serious illness, you may need extra help with
day-to-day tasks. You may even need an in-home medical visit to

Educates and supports young adults and helps prepare them

Sports Physical
Helps new and pregnant mothers keep themselves and their
babies healthy. Participants will receive messages through
Text4Baby pregnancy and baby's first year. Each text contains health and
safety tips for topics like taking care of yourself before baby is
born, labor and delivery, breastfeeding, safe sleep, nutrition, and
car seat safety.
Assistance for rides to/from medical visits and the pharmacy, food
resources, Department of Social Services (DSS), and Weight
Watchers. Call MedTrans 1-833-215-3884 to make a reservations for
both urgent or non-urgent medical appointments. Must schedule 5
Transportation days in advance for non-urgent appointments.
