CCC Plus members - up to 14 days of meals provided to
Food Assistance
member safter hospital discharge. Call member services.
Gift cards for CCC Plus members who receive reminders for
Gift Card Rewards

Program Details
Healthy Heartbeats Program: Program to help you have a healthy
Pregnancy Support
pregnancy and a healthy baby.

Baby Showers: Have fun and learn about healthy pregnancy tips
and resources. There is a diaper bag raffle and all attendees get
a care package full of baby basics.

Text4Baby: Free text messages with health tips to assist you

through pregnancy, birth, and your baby’s first year.

Supplies/Services: Members have access to a free breast pump,

lactation consultant, prenatal classes, parenting classes, and
family planning.
Transportation provided for non-emergency medical services
Transportation and dental services covered by Smiles for Children. Call to
schedule ride 5 days before appointment. Call 1.855.880.3480
to schedule. Not covered for FAMIS members.
