AFFIDAVIT OF ARRESTING OFFICER

I, POLICE OFFICER III AUSTIN LAMBERT L. ANGELES, Filipino,


of legal age, single and bonafide member of the Philippine National Police,
presently assigned at Manila Police District, Ermita Police Station located
at Katigbak Parkway, Rizal Park, Barangay 653, Zone 68, Ermita, Manila,
do hereby depose and say that:

1. I am one the members of the Task Force on Anti-Illegal


Drugs who conducted entrapment operations at Diamond
Hotel located along Roxas Boulevard, Corner Dr. J. Quintos
Street, Manila;

2. Said operation stemmed from the complaint of JUAN A.


DELA CRUZ for violation of Republic Act No. 9165
(otherwise known as The Comprehensive Dangerous Drugs
Act of 2002) which was referred to our office by the
authorities against MELISSA B. SANTOS, Filipino, of legal
age, single and residing at Unit No. 1510 Vista GL Taft
Residences, Taft, Manila;

3. At about 5:00 PM of May 13, 2023, the undersigned


together with the complainant proceeded at the pre-
arranged sale of methamphetamine hydrochloride,
commonly known as "shabu" amounting to FIVE
HUNDRED THOUSAND (P500,000.00) PESOS, Philippine
Currency at Diamond Hotel located along Roxas Boulevard,
Corner Dr. J. Quintos Street, Manila, to give the money
being asked by the respondent;

4. Upon arrival thereat, complainants waited for the suspect to


arrive while the undersigned seated near the table of the
complainants. At about 7:00 PM, the suspect arrived and
approached the complainants. After a few conversations,
the complainants handed over the marked money to the
suspect and minutes prompted the undersigned to
immediately effect the arrest;

5. Seized and recovered in the possession and control of the


arrested person were the following, to wit:

a. Marked Money;
b. iPhone 13 Pro Max and SIM Card; and
c. Other evidence confiscated;
6. Suspect together with the seized and recovered evidence, was
to the Ermita Police Station for booking and proper disposition.
Likewise, the arrested person was apprised of her rights as
provided for under the Constitution of the Philippines.

7. Respondent was suspected to be involved in the sale and use of


Methamphetamine Hydrochloride, commonly known as “shabu”,
which according to District Chief Major James E. Adan a
produced positive determination of the illegal activities of the
accused;

8. Upon Major Adan's request, Lt. Elizabeth B. Reyes, Forensic


Chemist, conducted physical, chemical and confirmatory tests on
the specimen submitted by Major Adan, and based upon said
tests, Lt. Reyes concluded that the specimen submitted was
Methamphetamine Hydrochloride or shabu. She reduced her
findings into writing, an initial report and a final report, wherein
she stated that “qualitative examination conducted on the
abovementioned specimen gave a POSITIVE result to the test
for Methamphetamine Hydrochloride (shabu), a regulated drug".
