AFFIDAVIT OF NO PENDING CRIMINAL INVESTIGATION

and/or PROSECUTION

            I, JAMES MATTHEW A. CRUZ, of legal age, single, Filipino, and presently

residing at Vista GL Taft Residences, Taft, Manila, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I was formerly employed as Administrative Officer in the AAA Group of

Companies, Inc.;

2. That I am retiring under Republic Act No. 1616, Presidential Decree No. 1146,
Republic Act No. 660 effective 30 June 2023.

3.  That there is no administrative criminal investigation against me for any offense,

defined or penalized under Republic Act No. 3019, otherwise known as the Anti-
Graft and Corrupt Practices Act, as amended and under the provisions of the
Revised Penal Code; and

4.  That I am executing this affidavit to attest to the truth of the foregoing facts or
assertation in compliance with Commission on Audit (COA) Memorandum No.
451 dated March 6, 1973.
