Scenario 4: Calling a Meeting in Response to an Incident

Canco Pharma has just called you; they have had a malfunction with the
automatic control system on one of their manufacturing lines. You are going
over there right now; it’s 15:00. The plan is to identify and replace the
components that aren’t working so that production won’t be interrupted for
too long. You have a technical crew meeting you there with some spare
components that you keep in your warehouse; you’re hoping that you’ll have
what is necessary to make an immediate repair.
However, first thing tomorrow morning you want to meet with the system
designers to see what the ultimate cause of the problem was, to determine
whether it is likely to recur, and what you might have to change so that it
won’t. You want to make the system more resilient. You decide to write an
email: the people you request to come to the meeting are Joe Ruth, Mike
Meausette, and Bill Kleepstra. They should bring the design plans. You will
update them later today on what you find at the Canco facility, but they
should do a little research about similar failures.
“First thing in the morning” means before the normal start of day; you’d
like them to meet you in the conference room on the third floor of your
office building at 07:00, but you will swing by Tim Hortons to pick up
coffee and bagels.
