In reference of the DBM-DOH Joint Circular 2022-0001 stipulating the guidelines on the grant of one
COVID-19 Allowance (OCA) to Public and Private Health Care Workers (HCWs) and Non-HCWs in Health
Facilities Involved in COVID-19 Response, we are respectfully sending our letter of intent for the
registration of our facility, Rural Health Unit Gabaldon as a health facility, we are humbly providing
Covid-19 services such as direct management of Covid-19 patient, contact tracing, screening, swabbing,
management and referrals if needed as well as the roll -out of the Covid- 19 vaccines.
