AFFIDAVIT COMPLAINT
I, _________________________________, of legal age, married, Filipino
Citizen with residence and postal address of No.
___________________________, Baguio City, Philippines, after having been
sworn to in accordance with law, hereby depose and sate:

1. That I am formally filling a complaint against

______________________________with residence and postal address
at No. __________________________, Baguio City, the particulars of
the charge are as follows:

a. That sometime on September 20, 2000,

____________________________ and I were married at Baguio
City before the Regional Trial Court of Baguio City. A Copy
of the Certification from the Office of the Local Civil
Registrar of Baguio City is hereto attached as annex “A” to
form part hereof;

b. That subsequently on October 19, 2001, we again

reaffirmed our marriage before the Iglesia ni Cristo Church,
Magsaysay Avenue, Baguio City. A copy of our certificate
of marriage is hereto attached as annex “B” to form part
hereof;

c. That during our marriage, ______________________ and

___________________________ were born as a fruit of our
marriage. ____________________ was born on March 12,
2005 and _______________________ was born on April 12,
2003. A copy of the Birth Certificates of our children are
hereto attached as annexes ”C” and “D” is hereto attached
is hereto attached to form part hereof;

d. That at the latter part of the year 2002 while we were still
living together as husband and wife, I noticed that ________
had been frequently leaving our home allegedly for so
many reasons which forced our my parents to take care of
our children for custody every time I go out to look for
employment;

e. That after sometime, my husband comes home rarely and I

later on discovered that every time he comes home he
carries most of his personal belongings until such time that
he almost took all his properties from our residence;

f. That from that time on, my husband failed to appear to our

residence and abandoned me and my children. Thus, I was
forced to seek support from my parents for the basic needs
of my children;

g. That after sometime I discovered that my husband is

currently employed with Texas Instruments;
 h. That up to this moment my husband never appeared nor
extended support to me and our children;

2. That I am executing this affidavit for filing a case for

ABANDONING A MINOR defined and penalized under article 276
of the Revised Penal Code of the Philippines against
______________________________________. Further, to attest to the
truthfulness of all the foregoing and for all legal intents and
purposes it may serve.
