COPYRIGHT AND DATA PROCESSING CONSENT FORM

This serves as confirmation that PARTICIPANT has given his/her consent and agreement for UNILEVER
PHILIPPINES, INC. (“UNILEVER”) to perpetual use of any photograph submitted by PARTICIPANT for the
PRIDE MONTH CAMPAIGN (“Campaign”) that includes PARTICIPANT, as well as any and all statements
made by PARTICIPANT and other personal information of PARTICIPANT and/or other person(s), without
need of securing additional express written permission. UNILEVER may use the photograph(s) and
relevant personal information in publications or other media platforms, used or contracted by UNILEVER
for the sole purpose of the Campaign, including but not limited to social media, websites, etc.

To be clear, PARTICIPANT acknowledges, agrees and confirms that s/he has read the attached Privacy
Notice, and hereby agrees to the perpetual use of any and all material that PARTICIPANT has submitted
for the Campaign, that contains their image and other personal information, for UNILEVER’s purposes,
including digital posts, and any and all future advertising materials and digital material, for UNILEVER’s (or
any of its authorized partners or personnel) use in any channel.

PARTICIPANT likewise warrants that by submitting the photograph/s to the Campaign, they have likewise
secured the consent of any other person(s) whose image(s) is also in the photographs for UNILEVER to
perpetually use the photographs and process their personal information for the sole purpose of the
Campaign, without need of securing additional express written permission.

Further, by signing this form, PARTICIPANT agrees that s/he grants UNILEVER (or any of its authorized
partners or personnel) a perpetual, irrevocable, worldwide, royalty-free, sub-licensable and transferable
right to use, reproduce, distribute, and make derivative works of any resulting copyright from the material
submitted by PARTICIPANT for the sole purpose of the Campaign, in any media and through any media
channel.
