REQUEST FOR EXHUMATION

To: Fr. Jose Fortunato J. Garces,

I, the daughter CHERRY ANN O. TAN, of legal age, Filipino, with residence and
postal address at 2057 Mindanao Avenue, Sampaloc, Manila, do hereby request for the
exhumation and transfer the ashes of NORAIDA O. TAN who has been buried since July
10, 1997.

The deceased niche rental of 25 years has been expired as per the Niche Contract
owned by my father, WILLIE TAN, the deceased widower.

It is our desire to exhume the ashes/urn of our deceased beloved and be transferred to
Manila North Green Park, Manila.

The lessor of the lot has freely given his consent and conformity to me, his daughter
for the said exhumation.
