teachers/professors, and other persons to provide factual and accurate information they have about

With regard to the foregoing disclosures, I hereby release all concerned from any liability in connection
therewith. I also understand that I may withhold my permission and that in such a case, no
investigation will be done, and my application for employment will not be further processed.
