SERVICE CONTRACT

This Employment Contract is executed and entered into by and between:

who bind themselves to the following terms and conditions:

1. That the Employer hereby hires and employs the Worker for a period of ______ days starting on
__________________ until ___________________;

2. That the Employer shall pay the Worker in cash the equivalent of the existing minimum wage
amounting to Php_______ per day for the total of Php__________ when the work is completed;

3. That the Worker binds himself to perform the tasks/duties assigned to him/her and strictly adhere
and observe the rules and regulations and/or policies prescribed by the Employer;

4. That the Employer reserves the right to terminate this appointment after due process if found
unsatisfactory performance of the assigned tasks, violation of policies and Labor Code of the
Philippines;

5. That the Employer may postpone or discontinue the services temporarily for a certain hours or
days for causes such as inclement weather, lack of materials and other causes beyond the control
of the Employer; and

6. That the Worker must fully understand that the employment is only for the short period, thus the
services shall be terminated upon completion of the project when there is no more work to do or
services are no longer needed. (Attached is the job description / function / responsibilities of the
Workers)
