LAW STUDENT PRACTITIONER (LSP) EXTERNSHIP AGREEMENT

I, __________________________, of legal age, Filipino, a Law Student Practitioner and a


Juris Doctor Student of University of St. La Salle College of Law (USLS COL), hereby
depose and state that:

1. I am a duly Certified Level 1 Law Student Practitioner (LSP) having been


certified last ____________________;

2. I acknowledge that the Clinical Legal Education Program (CLEP) is one of the
subjects of the Juris Doctor Curriculum approved by ___________, and a
mandatory requirement before taking the bar examinations under SC AO No.
_____ dated ______ and LEB Res. No. ____________ dated ___________.

3. As part of CLEP, I voluntarily agree to undergo and participate in the Externship


Program (Program) of the USLS COL;

4. For the duration of the Program, I hereby promise to perform diligently the
responsibilities of a LSP, to wit:

a) to abide by the administrative policies, rules, and standards of USLS COL


as provided by the USLS Student Handbook;

b) to accomplish and submit a Law Student Practitioner Profile to the


Externship or Clinic Professor;

c) to accomplish and submit reports or other documents as required by the


Externship or Clinic Professor;

d) to report to my assigned Supervising Lawyer as often as determined by


them;

e) to accomplish the assigned tasks to the best of my abilities and conform


with the Program set forth by USLS COL and the Externship Host;

f) to complete the agreed scope of work within the agreed duration of the
Program; and

g) to timely report for training and promptly accomplish assigned tasks or


assignments in the best of my ability and in view of my expected skill and
knowledge of the subject matter;

h) To voluntarily submit myself to academic evaluation based on CLEP


metrics for purposes of determining my graded corresponding to my
performance under CLEP 1 and CLEP 2 subjects.

5. I renounce and waive any claim against Externship Host and/or USLS COL for
any injury or loss that I may sustain or may suffer, personal or pecuniary, in the
performance of my duties and functions while under training, unless such is

attributable to the negligence or fault of any of the employees, officers, or
personnel of the Externship Host and/or USLS COL;

6. I shall be personally responsible for any and all liabilities and/or injuries to myself
or to property or to persons attributable to my own fault or negligence while in
the course of my training. In this light, I shall hold the Externship Host and USLS
COL free and harmless from any demand, claim or complaint, whatsoever arising
from this training agreement, except in cases of negligence, malicious acts and
criminal acts by any of the officers, employees or agents of the Externship Host
or USLS COL;

7. I understand that all information gathered by the LSP on the operations and legal
matters of the Externship Host are classified as confidential in nature. I hereby
undertake to prevent transfer of such information by any of its members,
consciously or unconsciously, to any party outside of the Externship Host
without the latter’s knowledge and written consent;

8. Upon the termination of externship from the Externship Host, I shall return to
the Externship Host all of his/her documents and property, including but not
necessarily limited to: reports, manuals, correspondence, client lists, case files,
and all other materials and all copies thereof relating in any way to the
Externship Host’s operations, or in any way obtained during the course of
externship;

9. I acknowledge, understand, and promise that I will comply with our Honor Code
– true to the Canons of Professional Responsibility, Law Student Practitioner’s
Oath of Office and the rules and guidelines set by the USLS COL for the CLEP.
