PREPARING MEDICATION FROM VIAL


Medications can come in different forms, such as ampules, vials, tablets, capsules, and so forth.
When withdrawing medication from a vial, there are a few things you'll want to know as a nursing
student or nurse. Today I’m going to demonstrate on how to prepare medication from vial.

STEPS RATINGS
First, we need to perform the assess the patient as a preparation for the 3 2 1
procedure
1. Assess if the patient is experiencing allergies to the medication
2. Inform them about the specific drug action, side effects,
interaction, and adverse reactions
 During treatment, knowing what adverse effects may
occur equips a patient to look out for possible problems,
and to deal with them appropriately if they occur.
3. Ask if they have knowledge of the medication and procedure.
 If they have not obtained knowledge about the medication
or procedure, you can give information about the
medication they’ll be taking and what are the procedures
or medication they’re receiving.
4. Check the Intended route of parenteral medication to determine
the appropriate size of syringe
5. Now we are going to check the Ordered ampule medication for
clarity and expiration date.
The ampule is… and the exp is about …
This means that the ampule is still consumable by this time.
6. The next thing we are going to do is to prepare the materials/
equipment needed.
 Client’s MAR/ Medication Card
 Vial of sterile medication what I have here is powdered form
 Antiseptic swabs
 Syringe
 Sterile water or normal saline, if drug because what we have
here is powdered form
SKILLS
7. Now let’s check if the ordered medication in the MAR is the
same as the ampule that we have.
So the ampule is …. and also let’s check the dosage that
the doctor ordered in the MAR which is 3cc or 3ml,also
check the route, and check if we are giving it at the right
time, to the right patient.

 Check the label on the vial carefully against the MAR


to make sure that the correct medication is being
prepared.
 Follow the three checks for administering medication.

8. Perform hand hygiene and observe other appropriate infection


prevention procedures.
9. Prepare the medication vial for reconstitution
 We need Read the manufacturer’s direction
 We have to get 0.9 from sterile water.
 (Insert and withdraw)
 Flick if there are bubbles.
 Get the vial and Remove the protective cap
 Turn invertedly, withdraw
Withdraw an equivalent amount of air from the vial
before adding the diluent, unless otherwise indicated by
the directions
 Add the amount of sterile water or saline (diluent)
indicated in the direction into the vial.
 Mix the solution, by rotating the vial between the palms
of the hands not by shaking to avoid the accumulation of
air.
10. Withdraw the medication
 Ensure that the needle is firmly attached to the syringe
 Remove the cap from the needle, then draw up into the
syringe the amount of air equal to the volume of the
medication to be withdrawn which is 3cc.
 Carefully insert the needle into the upright vial through
the center of the rubber cap, maintaining the sterility of
the needle.
 Invert the vial
 Inject the air into the vial, keeping the level of the needle
above the surface of the medication
 Withdraw the prescribed amount of medication which is
again, 3cc
 Hold the syringe and vial at eye level to determine that
the correct dosage of the drug is drawn into the syringe.
 (KEEP IT INVERTED)
 Eject air remaining at the top of the syringe into the vial.
 When the correct dosage volume of medication plus a
little more is obtained, withdraw the needle from the vial,
and replace the cap over the needle using the scoop
method thus maintaining its sterility.
11. If necessary, tap the syringe barrel to dislodge any air bubbles
present in the syringe
12. If giving an injection replace the needle with a new one of the
correct gauge and length. Eject air from the new needle and
verify correct medication volume before injecting the client
13. If multidose vial is reconstituted, label the vial with the date and
time it was prepared, the amount of drug contained in each
millilitre of solution and your initials.
14. Once the medication is reconstituted, store it in a refrigerator or
as recommended by the manufacturer
